Radiologic Analysisof Pancreas Carcinoma




Abstract
34 cases of pancreas carcinoma which was proven by operation and biopsy during, last 4 ¨ö years from Jun. 1978 to Dec. 1982.
The results were as follow;
1. Pancreatic carcinoma are most frequently occurred in patient within the 40-69 age range (84. 8%). Male are somewhat more commonly affected (M : F=1.6 : 1).
2. Major clinical findings are epigastric pain (41. 2%), weight loss (29. 4%) jaundice (26.5¡Æ0) , palpable mass (23.5¡Æ0) and loss of appetite (14.7%).
3. 20 cases (58.8%) involve head of pancreas, 13 cases (38.2%) body and tail and one case diffusely.
4. Histollogically, adenocarcinoma occurred most frequently which was 32 cases (94.1¡Æx) cystadenocarcinoma and pancreaticobl astoma occurred in one cases respectively.
5. Radiologic Findings.
1) U.G.I. study
U.G.I. was performed in 21 cases of pancreas carcinoma, 12 out of 14 cases (85.8%) of pancreas head carcinoma and 6 out of 7 cases ;85.7%) showed positive findings.
2) Percutaneous transhepatic cholangiographies were done in 8 cases, which show abrupt obstruction at distal CBD. The appearance of obstructions are trapered nipple in 3 cases, bird beak in 2 cases, rat tail in 2 cases and intraluminal polypoid lesion in one case. Coincidentally performed 7 cases of hypotonic duodenography showed obstructon at distal common bile duct at the level of superior margin of pancreas, associated with mass effect on medial aspect of second duodenum. Catheter biliary drainages were performed in 3 cases.
3) 11 cases of endoscopic retrograde cholangiopancreatography were done with 3 cases failure. Two cases were failed due to stenosis of proximal second duodenum and one case cannulation failure
Findings of pancreatic duct in 8 cases of ductal - visualization were abrupt obstructive type in 6 cases, stenotic type in one cases and no ductal change in one case.
4) 15 cases of ultrasonography were done, 9 case in head of pancreas and 6 cases in body and tail carcinoma.
Ultrasonogram findings of head` of ¢¥pancreas carcinoma were mass lesion in 3 cases, mass lesion with common bile duct dilatation in 4 cases and only common bile duct dilatation in 2 casea 5 out of 6 pancreatis body and tail carcinoma showed mass lesions,
5) C-T scan was performed in 4 patient with pancreatic mass in 3 cases. Ing one case liver metastasis was detected.
6) Angiograpy was done in 2 cases.
One case showed no change. The other case showed splenic artery encasement and splenic vein occlusion with multiple gastric varices without esophageal varices.
